Please fill out the contact form below. If you are an artist, please include in the message box your Social Media handles so we can view your portfolio. If you are a vendor or sponsor, please include the name of your company. We will reach out to you once we review your application.

This is a preliminary application. Once submitted, you will be contacted and provided a full application form, including a contract and payment details. You will be provided more Information for the Nebraska Department of Health and Human Services Body Art Profession Tattoo Artist License.

All tattoo artists that will be participating will be required to provide a copy of their 2026 NDHHS Tattoo Artist License. This document can be sent via email, prior to September 1st, 2026.
